Pre-Registration

If your physician has scheduled you for surgery, a test or another type of inpatient or outpatient service at Southern Regional Medical Center or Spivey Station, you may use this site to pre-register for your visit. You must pre-register online one full business day (24 hours) prior to services.

Our goal is to make the admission process easier for you by receiving your registration information prior to your visit. Your pre-registration form should be completed as soon as you are scheduled for your visit.

Southern Regional recognizes the confidential nature of your registration information, and the form you complete online will be secure. Once you have submitted your preregistration form, you will receive an e-mail confirmation. Click on the link below to begin.

Pre-Register Online

  1. Please allow up to 15 minutes to complete the form. You will need to have the patient's information ready, including:
    • The patient's occupation, place of employment, and the employer's phone number
    • The patient's insurance information
